Beriberi Heart



Abstract
Among 800 necropsies, 8 cases of beriberi heart were observed. Six of them were female and the onset of their illness was closely related to the recent delivery.
Major clinical signs were congestive heart failure, characterized by dyspnea and generalized edema. Cardiac signs were irregular murmurs and diffuse cardiomegaly, but the electrocardiographic pattern was inconstant.
Pathologic findings of the heart were hydropic degeneration, myocytolysis, hypertrophy, hyaline degeneration and necrosis of myofibers: various degree of edema, mild leukocytic infiltrations and irregular fibrosis of the interstitium; and fibrous thickening with frequent mural thrombosis of the endocardium. Among these findings, the most interesting and hitherto unrecognized were frequent and extensive necrosis of subendocardial myocardium without evidence of vascular occlusion or active inflammatory reaction, and subsequent mural thrombosis.
